Keeping your details up to date – It is important we have your correct information – Let us know if you change any of the following: address, telephone numbers, emergency or next of kin details, Medicare care and pension or health care card details.
Our practice is committed to preventative care; we will provide you with a reminder notice from time to time offering you preventative health services appropriate to your care.

Doctors@Bayside offers home visits to patients who are unable to attend the clinic due to extreme circumstances. These visits however, are not bulk billed and will incur a private fee of $200. Payment is required prior to attending.
CARE OUTSIDE NORMAL OPENING HOURS ARRANGEMENTS
Monday to Friday after 11pm, and Saturday from 12pm until 08:30am Monday morning, our afterhours locum service, DoctorDoctor, is available. Our phones are diverted to their number, or they can be contacted on 13 26 60.
If seen by a locum doctor, a report will be forwarded to the clinic on the next business day and will be added to your file.
Doctors@Bayside endeavours to accommodate all patients’ requirements regarding appointments and every effort will be made to accommodate your preferred time and health professional. Emergencies are given priority and our support team will attempt to contact you if there is any unforseen delay or your GP has been called away. Patients are welcome to walk in and see the next available GP, a choice of GP is not offered for this service and waiting times can vary according to the day’s appointments. Longer consultation times and interpreters are available if needed. Online appointment system can be assessed form our website.
If your health does not improve as the doctor has expected or if you are worried about your condition, please do not hesitate to contact our Practice Nurse. Doctors do not take calls from patients whilst consulting. If you wish to leave a message for a doctor, all messages are passed onto doctors, if required they or a staff member will call you back.
MANAGEMENT OF YOUR PERSONAL HEALTH INFORMATION
Your medical record is a confidential document. It is policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of our team.
If you have a problem we would like to hear about it. Please feel free to talk to your doctor or our Practice Manager. You may prefer to write to us or use our suggestion box. We take your concerns, suggestions and complaints seriously. As a professional courtesy all complaints should be directed to the practice initially.
A follow up appointment with a doctor will be required to obtain your test results. Please make an appointment with your GP at the front desk for 2-3 days after completion of all tests. If a result needs to be dealt with earlier, our Practice Nurse will contact you to come in earlier.
Keep us informed of changes to your health, this can include updating you family and social history, a change to allergies from food or medication. Changes to your personal details such as; address, telephone numbers, emergency and next of kin details. These things are important, so let us know as soon as you become aware of such change.
